Evidence for this report was heard from Railtrack, the Office of Passenger Rail Franchising, the Rail Regulator, the Department of Transport and the Health and Safety Executive.

It highlights the principal railway safety issues that arose in the oral evidence sessions.

The conclusions and recommendations cover: slam doors; level crossings; Railtrack's regard for the safety implications of new rolling stock; replacement/conversion of Mark 1 coaches; state of disrepair at Euston; inadequate maintenance of the Forth Bridge; automatic train protection; transmission-based signalling equipment for the West Coast mainline; and cab-secure radio.
