Cystic Fibrosis Transition Programme for Teens

This programme was designed by an adult who suffers from cystic fibrosis, to help teenagers' transition smoothly into adult CF care.

By following the programme, each patient will learn, step by step, how to take full responsibility for their own health.

It is a gradual process, so patients can progress slowly and feel confident before moving to adult care.

Parents can also follow their progress and therefore, slowly decrease the responsibility they have over their teen's health.

This programme should in turn make life easier for the new CF team, when dealing with teens, as they will be ready for adult care.
